Physician Assistant (PA) - Chronic Care Management & Remote Patient Monitoring

Position Summary

AtTeleMateHealth, we are dedicated to transforming healthcare delivery through innovative solutions. We deliver individualized, holistic patient care while connecting in a waythat'seffective for the patient.We are seeking a compassionate and patient-focused Physician Assistant (PA) to join our growing care management team.

As a TeleMate Health PA, you will conduct initial patient assessments, develop individualized care plans, provide chronic disease and medication management education, and support patients enrolled in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. The position will include follow-up and as-needed visits to help patients achieve better health outcomes and successfully manage their chronic conditions. This position is primarily remote, with travel local to the provider.This position offers flexibility, work-life balance, and the opportunity to make a meaningful impact on patients' lives.

Responsibilities

  • Conduct comprehensive initial patient visits, including:
    • Review of medical history and diagnoses
    • Medication reconciliation and review
    • Assessment of patient needs and health goals
    • Development and discussion of individualized care plans
  • Evaluate patients for enrollment and participation in RPM and CCM services.
  • Determine appropriate monitoring equipment and patient support needs.
  • Provide education regarding chronic disease management, medications, lifestyle modifications, and treatment plans.
  • Conduct follow-up and PRN visits to address patient concerns, medication questions, changes in condition, and care plan updates.
  • Collaborate closely with care managers, nurses, providers, specialists, pharmacists, and other members of the healthcare team.
  • Identify barriers to care and coordinate appropriate resources and interventions.
  • Document patient encounters accurately and in compliance with regulatory and billing requirements.
  • Support quality initiatives focused on reducing hospitalizations, improving patient engagement, and enhancing overall health outcomes.

Qualifications

  • Graduate of an accredited Physician Assistant program.
  • Current Physician Assistant license in good standing.
  • National certification as a Physician Assistant.
  • Strong clinical assessment and patient education skills.
  • Experience in primary care, internal medicine, family medicine, geriatrics, cardiology, or chronic disease management preferred.
  • Familiarity with Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs is preferred but not required.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
