Work Arrangement: Fully Remote

Targeted States: CA, OR, WA, AZ, NM, NV, UT, ID, MT, WY, KY, OK, AR, LA, WV, NH, VT
Licensure Requirement: Active license in at least one of these states is required.


Role Overview

An innovative telehealth organization is seeking a Pharmacist to support patients across multiple Western and Central U.S. states through remote clinical services.

This role focuses on virtual patient care delivery, emphasizing medication management, education, and coordination with healthcare teams to drive better clinical outcomes.

Ideal candidates are proactive clinicians who are comfortable leveraging technology to enhance patient care.


Key Duties

  • Perform Medication Therapy Management (MTM) interventions
  • Complete medication reconciliation and identify optimization opportunities
  • Provide patient-centered education on medications and disease management
  • Support chronic care initiatives (e.g., diabetes, hypertension, COPD, CHF)
  • Use behavioral techniques such as motivational interviewing to improve adherence
  • Collaborate with providers on treatment planning and medication adjustments
  • Detect and resolve medication-related risks and discrepancies
  • Ensure timely and accurate documentation of all encounters
  • Facilitate smooth care transitions and ongoing care coordination


Required Qualifications

  • Doctor of Pharmacy (PharmD)
  • Active pharmacist license in at least one of the following states:
    CA, OR, WA, AZ, NM, NV, UT, ID, MT, WY, KY, OK, AR, LA, WV, NH, VT
  • Open to obtaining additional licensure as required


Preferred Background

  • Board certification (e.g., BCPS, BCGP) preferred
  • Experience in:
    • MTM delivery
    • Ambulatory or outpatient care
    • Chronic disease management programs
    • Patient engagement and education
  • Familiarity with programs such as CCM, TCM, RPM is beneficial
