• Manage an independent caseload of patients, conducting thorough evaluations and assessments to identify psychosocial, emotional, financial, and cultural factors impacting patients' mental health and treatment plans.
• Craft and implement evidence-based treatment plans tailored to address the unique needs of high-risk patients, acting as a primary advocate for their rights and well-being.
• Provide crisis intervention for patients and families, ensuring timely response to complex psychosocial issues that arise during treatment.
• Lead communication efforts with the healthcare team to ensure that all educational, resource, and referral needs of patients and their families are met effectively.
• Supervise and guide junior social work staff, interns, and volunteers, sharing insights that contribute to the continuous improvement of social work practices within the organization.
• Coordinate with community resources to facilitate seamless care transitions that minimize any risk of hospital readmission.
• Respond promptly to psychosocial services needs, maintaining efficiency that aligns with patient discharge timelines.
• Participate in multi-disciplinary rounds and specialized patient care conferences to remain aligned with case management objectives.
• Stay current on legislation and policies affecting mental health service delivery and advocate for integration within care coordination systems.

In this role, you will primarily engage with individuals undergoing detoxification and those facing serious mental health challenges as a result of substance use. Our patient population ranges from adolescents to adults, often exhibiting complex needs that require nuanced and culturally competent care strategies. Your expertise will help them transition safely and effectively through their recovery journey.

• Master's Degree in Social Work (MSW) from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.
• A minimum of two years of clinical experience in a healthcare setting, preferably working with a similar patient demographic.
• Strong knowledge of social work principles, theories, methods, and ethics related to mental health services.
• Exceptional communication and interpersonal skills, with the ability to navigate sensitive situations with empathy and professionalism.

• Experience in detoxification services or addiction recovery.
• Familiarity with case management processes and interdisciplinary teamwork.

Memorial Care offers a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and qualifications.
