About the Role:
Care Essentials Home Health is looking for a compassionate and dedicated Case Manager to join our growing team in San Bernardino. In this role, you'll make a meaningful difference in patients' lives by coordinating high-quality home health services that support recovery, independence, and well-being. Responsibilities:
  • Assess, plan, and coordinate individualized care plans for home health patients
  • Serve as the primary point of contact between patients, families, and multidisciplinary care teams
  • Monitor patient progress and adjust care plans to meet evolving clinical needs
  • Coordinate referrals, authorizations, and community resources to ensure continuity of care
  • Conduct home visits and telephonic follow-ups to evaluate patient outcomes
