• Participate in weekly care team huddles to enhance interdepartmental collaboration and patient-centered planning.
• Conduct thorough assessments and evaluations to develop effective mental health treatment plans tailored to individual client needs.
• Deliver individual and group therapy sessions that focus on promoting mental wellness and achieving treatment goals.
• Collaborate with medical providers during patient appointments to address psychosocial issues affecting overall health and well-being.
• Manage triage of both urgent and routine referrals from medical professionals, ensuring timely care for clients requiring immediate intervention.

You will work with a diverse range of clients, addressing various mental health challenges, including anxiety, depression, trauma, and relationship issues. • A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.
• Current licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Social Worker (LCSW) in the state of Pennsylvania.
• Demonstrated ability to manage time effectively, prioritize tasks, and maintain organization in a dynamic and often fast-paced environment.
• Strong interpersonal skills, allowing for effective rapport-building with clients and collaboration with healthcare teams.

• Experience providing telehealth services, with a strong comfort level using virtual communication platforms.
• Prior work in integrated care settings, collaborating closely with primary care providers.
• Expertise in creating and implementing treatment plans that foster client engagement and measurable progress.

The position offers a competitive salary ranging from $90,000 to $100,000 annually, commensurate with your experience and caseload.
