This position is primarily remote, allowing our LCSW to work from the comfort of their home while engaging with clients and families through virtual platforms. The Licensed Clinical Social Worker will be instrumental in delivering care and support to children and families navigating challenging circumstances. Key responsibilities include:

• Conducting thorough evaluations and assessments to gauge the individual needs of children and families in hospice care.
• Participating in the development and implementation of Individualized Family Service Plans (IFSPs) that support children’s growth and development.
• Monitoring outcomes and making necessary adjustments as a proactive team member, ensuring that each child receives the best possible care.
• Providing emotional and logistical support to families, empowering them through education on available resources, coping strategies, and development-related skills.
• Complying with all federal and state regulations to ensure the highest standard of practice.
• Building rapport with clients by fostering a trusting and supportive relationship.
• Implementing evidence-based practices in alignment with natural learning environment strategies.
• Ensuring safety and health standards are met within service delivery.

The LCSW will primarily serve children experiencing terminal or life-limiting conditions and their families. The ideal candidate for this position will possess the following qualifications:

• A Master’s Degree or Doctorate of Social Work, along with a valid license issued by the Department of Public Health.
• Active LCSW certification is required.
• A valid driver’s license to facilitate home visits and community outreach as needed.
• A strong understanding of diverse learning styles and the ability to tailor interventions accordingly.
• Excellent organization and time management skills.
• Exceptional communication skills, both verbal and written, with an emphasis on empathy and understanding.
• A commitment to fostering collaborative relationships within a multidisciplinary team.

While not mandatory, candidates with the following experience will be preferred:

• A minimum of one year working with children in early intervention settings.
• Experience utilizing Natural Learning Environment Practices and implementing child development strategies.
