This role is designed to be fully remote, allowing for flexibility in managing your schedule while meeting the demands of client care. You will interact with clients primarily via virtual platforms, but will occasionally visit clients in their homes as needed for assessments or well-being checks. The community health clinic environment fosters collaboration with interdisciplinary teams, including nurses and caregivers, providing a network of support to enhance service delivery. Our work hours typically span from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend availability required to meet client needs.

As a Licensed Master Social Worker, you will undertake a variety of critical tasks:

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors that affect their daily lives.
• Collaborate with clients and their families to create tailored care plans that address individual goals, preferences, and support networks.
• Provide ongoing counseling and emotional support, guiding seniors and their families through transitions such as bereavement, health crises, or relocations.
• Advocate for clients’ rights and assist them in accessing community resources, ensuring they receive necessary services and support.
• Work alongside an interdisciplinary team to coordinate seamless care, including attending team meetings and case reviews.
• Perform regular client visitations to monitor their progress, evaluate living conditions, and proactively address any potential concerns.
• Facilitate communication among family members involved in the care of senior clients, helping them navigate conflicts and enhance collaboration.
• Educate clients and families about available resources, support groups, and activities to promote social engagement and reduce isolation.
• Document all client interactions, assessments, and care plans meticulously in our electronic health record (EHR) system to maintain compliance and quality oversight.
• Stay updated on industry practices, policies, and ethical standards to ensure the highest quality of care.

This position primarily focuses on seniors and their families, who often face complex challenges related to aging, including chronic health conditions, mental health issues, and social isolation. As an LMSW within our community health clinic, you will work closely with this demographic, applying best practices in gerontological social work to improve their quality of life and health outcomes.

• A Master's degree in Social Work (MSW) from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of experience in a social work capacity, ideally within a community health or geriatric setting.
• Thorough understanding of social work principles, aging-related issues, and local community resources.
• Exceptional assessment, counseling, and problem-solving skills.
• The ability to connect with seniors and their families, demonstrating empathy and cultural competence.
• Strong communication skills, both verbal and written, with the ability to collaborate effectively in an interdisciplinary setting.
• Proficiency in using electronic health record (EHR) systems and other relevant computer applications.

• Experience working in home health, community health agencies, or long-term care facilities.
• Previous involvement with case management, care coordination, and multi-disciplinary team approaches.
• Knowledge of local, state, and federal regulations regarding social work practice.
