• Conduct comprehensive psychosocial assessments to understand clients' emotional, mental, and social needs, as well as their personal history and challenges they face while in detox.
• Develop and implement personalized treatment plans in collaboration with clients and their families that identify specific goals and track progress.
• Offer individual counseling and emotional support, helping clients address the complexities of addiction recovery, such as coping mechanisms and life transitions.
• Act as a strong advocate for clients, ensuring access to necessary community resources, including housing assistance, medical care, and support groups.
• Collaborate effectively with interdisciplinary teams, including healthcare professionals and addiction specialists, to synchronize treatment efforts and optimize care delivery.
• Monitor client progress through regular check-ins, adjusting treatment plans as needed based on feedback and evaluations.
• Educate clients and their families about available community resources, supporting their integration back into society and fostering long-term recovery.
• Maintain meticulous documentation in electronic health record (EHR) systems, ensuring compliance with regulatory standards and organizational policies.
• Stay updated on the latest trends and best practices in social work and addiction recovery to continuously improve the quality of care provided.

This role focuses primarily on individuals undergoing detoxification from substance use. Many clients may experience emotional distress, mental health issues, or social isolation as a result of their addiction. As a Social Worker, you will work to build trust and rapport, providing empathetic support and practical resources to enhance their recovery experience.

• Master's degree in Social Work from an accredited institution, ensuring a solid foundation in social welfare principles and ethics.
• A current and valid LMSW license issued by the state of Illinois, confirming your ability to practice professionally.
• A minimum of two years of relevant experience in social work, ideally within a community health or addiction-focused environment.
