As the Licensed Clinical Social Worker, your day-to-day tasks will include:

• Conducting thorough assessments to identify and evaluate the psychosocial needs of senior clients, incorporating their emotional, mental, and social factors into care plans.
• Collaborating with clients and their families to create tailored care strategies that respect their individual goals and preferences.
• Providing counseling and emotional support to assist seniors in navigating significant life transitions such as health changes, loss of independence, or the passing of loved ones.
• Advocating for clients' rights, ensuring they have equitable access to community resources and services crucial for their well-being.
• Working within interdisciplinary teams comprising nurses, caregivers, and other healthcare professionals to ensure seamless coordination of services and optimal client outcomes.
• Performing regular home and facility visits to monitor living conditions, assess client safety, and swiftly address any concerns that arise.
• Facilitating constructive communication and conflict resolution among family members involved in the care of the senior client.
• Proactively educating clients and their families about available resources and community programs that foster engagement and overall quality of life.
• Maintaining accurate and timely documentation of client assessments, care plans, and progress notes to comply with ethical standards and best practices.
• Staying updated with the latest developments in social work practices and regulations to enhance the quality of care provided.

Your primary focus will be on seniors, a demographic that requires specialized understanding and sensitivity due to the unique challenges they face, including health care complexities, social isolation, and mental health needs. You'll be instrumental in ensuring that their voices are heard and respected, while enabling them and their families to navigate community support systems effectively.

• A Master's degree in Social Work from an accredited institution is required.
• Active and valid licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.
• At least two years of experience in a social work role, specifically within a community center or geriatric setting.
• Proficient knowledge of social work methodologies, aging-related issues, and the array of resources available within the community.
• Exceptional communication and interpersonal skills to foster fruitful collaboration with clients and professionals alike.
• Strong assessment and counseling capabilities, allowing for effective problem-solving and support provision to clients in distress.
• Experience with electronic health record (EHR) systems is beneficial.
