Work Environment

This position is fully remote, allowing you to serve clients from the comfort of your own home while still receiving the necessary support and collaboration from our interdisciplinary team. Our work setting is designed to be flexible and accommodating, with a typical work schedule from Monday to Friday, 8:00 AM to 4:30 PM. However, occasional evening or weekend hours may be required based on client needs. You will have the opportunity to manage your time effectively and creatively address the unique needs of your clients.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments of senior clients, evaluating emotional, mental, and social factors that may affect their well-being.
  • Develop and implement personalized care plans in collaboration with clients and their families, outlining goals and strategies tailored to individual needs.
  • Provide individual and family counseling to help seniors navigate life transitions such as health changes, loss, or social isolation.
  • Advocate for clients’ rights and ensure they have access to vital community resources and support services.
  • Collaborate with other healthcare professionals, including nurses and caregivers, to coordinate comprehensive care and enhance service delivery.
  • Regularly monitor clients’ progress through scheduled visits or virtual check-ins to assess living conditions and identify any immediate concerns.
  • Facilitate family meetings to communicate care plans and address any conflicts regarding the client's well-being and needs.
  • Educate clients and their families about available resources, support groups, and programs aimed at enhancing social engagement.
  • Maintain meticulous and up-to-date documentation, including client assessments, care plans, and progress notes, ensuring compliance with ethical standards and confidentiality.
  • Stay informed on best practices and policy changes within the field of social work, particularly pertaining to geriatric care.
Patient Population / Client Focus

Qualifications
  • A Master’s degree in Social Work from an accredited institution.

We offer a competitive salary range of USD 60,000 to 100,000 annually based on experience and qualifications.
