The outpatient office serves as a hub for client interactions, assessments, and therapeutic sessions, providing a comfortable atmosphere for healing. • Assist primary care providers in recognizing and managing mental health and psychosocial issues by serving as an integral part of the primary care team.
• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and provide recommendations to primary care providers for treatment goals and plans.
• Deliver brief, targeted interventions for patients requiring mental health services, helping them navigate their emotional challenges.
• Evaluate which patients may benefit from continued care within the primary care setting versus those needing referrals to specialized mental health facilities.
• Engage in follow-up processes to assess patient progress and modify treatment approaches as necessary.
• Develop relapse prevention plans for patients, enabling them to maintain stability and healthy functioning.
• Identify ‘at risk’ patients and create proactive plans to avert worsening conditions.
• Facilitate the coordination of health services related to behavioral health care and collaborate with other treatment providers, ensuring patient consent for outside referrals.
• Document patient progress and diagnostic information in treatment charts, maintaining accurate records in compliance with legal and ethical standards.
• Advocate for patient needs by keeping primary care providers informed and collaborating on treatment plans.
• Support patients in achieving healthier lifestyle choices by addressing behavioral changes necessary for managing physical conditions.
• Empower patients by equipping them with self-management skills and educational materials to engage actively in their treatment.
• Provide consultation to clinic management and team members regarding behavioral services, focusing on program evaluation and outcomes.
• Assist patients in navigating medical treatments prescribed by primary care providers, offering coping strategies for any medication side effects.
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals.
• Perform additional responsibilities as assigned, contributing to the overall mission of the organization.

• A minimum of a Master's Degree in Clinical Counseling is required.
• Current LPC (Licensed Professional Counselor) License is mandatory.

• Familiarity with various therapeutic modalities, including family therapy, group therapy, and child psychotherapy.
• Bi-lingual proficiency in English and Spanish is preferred but not mandatory.
• Excellent understanding of the community resources available for mental health support.
• Ability to establish and maintain effective relationships with patients, families, and professional staff.
• Strong organizational skills for maintaining clinical records and reporting requirements.
• Experience working collaboratively within an interdisciplinary team is important for success in this role.

• Prior experience in outpatient clinical settings or multidisciplinary team environments is highly beneficial.
• Experience with telehealth platforms and documentation systems will enhance the delivery of care in a remote setting.
• Previous involvement with community health initiatives or extensive knowledge of specific mental health issues prevalent in the region would be advantageous.

This position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
