The Licensed Clinical Social Worker will have the following key responsibilities:

• Build and maintain trusting, therapeutic relationships with clients and their families, demonstrating dependability and empathy.
• Participate in multidisciplinary team meetings to discuss patient care plans, ensuring alignment with organizational goals.
• Conduct comprehensive assessments to evaluate clients’ emotional and psychological needs, utilizing evidence-based treatment plans tailored to each individual's circumstances.
• Develop and implement integrated care plans that outline interventions and services to address the needs of hospice patients and their families.
• Provide short-term therapeutic intervention and preventive care strategies, ranging from crisis management to long-term coping mechanisms.

• Collaborate with external services and resources to ensure continuity of care, addressing social determinants of health impacting patient outcomes.
• Engage in continuous professional development and training to enhance service delivery and personal growth.

This role focuses on hospice patients facing severe emotional and mental health challenges associated with terminal illnesses. To be considered for this role, candidates must meet the following qualifications:

• A Master's degree in Social Work (MSW) or a closely related field is required.
• Active LCSW licensure in the state of Texas is mandatory.
• Minimum of 2 years of clinical experience in a social work capacity, preferably within hospice, palliative care, or similar settings.
• Extensive knowledge of assessment, diagnosis, and treatment modalities relevant to the hospice population.
• Superior verbal and written communication skills, with an ability to engage effectively with clients of varying backgrounds.
• Strong organizational skills and the ability to manage multiple cases efficiently.

While the above qualifications are essential, the ideal candidate will also possess:
