Position Overview

Comfort Keepers Counseling Center is actively seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team in providing essential mental health services remotely. This full-time position will enable you to positively impact the lives of our clients by offering support and interventions tailored to their unique needs, specifically within a hospice care setting.

Key Responsibilities
  • Conduct detailed clinical assessments of clients to determine their mental health status and the most appropriate services needed.
  • Develop individualized treatment plans that address the unique emotional, psychological, and social challenges faced by hospice patients and their families.
  • Utilize evidence-based interventions and therapeutic techniques to support clients in navigating grief, anxiety, and other mental health issues related to terminal illnesses.
  • Thoroughly document client progress and responses to treatment in adherence to regulatory standards and internal protocols.
  • Engage in regular collaboration with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, participating in meetings to discuss client care strategies.
Patient Population / Client Focus

In this role, you will work primarily with clients who are in hospice care, which includes individuals dealing with terminal illnesses, their families, and caregivers. Your expertise will play a crucial role in helping them cope with the emotional and psychological implications of their circumstances, ensuring they feel supported and understood during this challenging time.

Qualifications
  • A current and active license as a Licensed Clinical Social Worker (LCSW) in the state of New York.
  • A Bachelorโ€™s degree in Social Work or a closely related field; a Masterโ€™s or Doctoral degree in Social Work is preferred.
  • Strong assessment, diagnostic, and treatment planning skills tailored to a hospice setting.
  • Effective interpersonal and communication skills, which are essential for building rapport with clients and collaborating with healthcare teams.
  • Organizational skills and time management ability to handle multiple cases while maintaining a high standard of care.
  • Familiarity with digital therapy techniques and remote work methodologies.
Preferred Experience
  • Minimum of two years of clinical experience, particularly with hospice or palliative care populations, is strongly preferred.
  • Experience working with individuals facing trauma, substance abuse issues, or varied mental health challenges is advantageous.
  • Proficiency with teletherapy platforms such as Zoom or Microsoft Teams, as well as electronic health record (EHR) systems.
