This full-time position allows for a remote work environment while primarily serving patients within a hospice center in Plattsburgh, New York. Key Responsibilities
  • Conduct detailed assessments to evaluate the severity, frequency, and duration of mood disorder symptoms in clients, employing formal assessment tools and interviews.
  • Collaborate with clients to formulate individualized treatment plans that are aligned with their specific goals, preferences, and overall mental health aspirations.
  • Utilize evidence-based practices, such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), to deliver therapeutic interventions tailored to client needs.
  • Educate clients and their families on mood disorders, treatment methodologies, and effective self-care strategies to promote empowerment and understanding.
  • Monitor clientsโ€™ progress through regular follow-up sessions, maintaining frequent evaluations and adjustments to treatment plans as needed for optimization of care.
  • Engage in collaborative efforts with psychiatrists and other healthcare providers to ensure comprehensive care pathways for clients with complex health needs.
  • Stay informed on the latest research and best practices concerning mood disorder treatment to enhance therapeutic efficacy.
  • Maintain meticulous and confidential documentation of all client interactions, assessments, and treatment updates, adhering to legal and ethical requirements.
  • Participate in team meetings and case consultations for peer support, sharing insights, and fostering a collaborative workspace focused on continuous quality improvement.
  • Actively advocate for clients, working to eliminate stigma surrounding mental health issues and ensure their emotional well-being is prioritized in patient care.
Patient Population / Client Focus

You will primarily work with clients experiencing mood disorders such as depression and anxiety, many of whom may be in hospice care or have complex emotional needs that require sensitive handling. The client demographic includes individuals from diverse backgrounds who may be experiencing significant life transitions, illness, or loss. Therefore, your ability to build rapport and adapt interventions to individual needs will be crucial in achieving positive therapeutic outcomes.

Qualifications
  • Master's or doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
  • Current and valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT) in New York State.
  • Demonstrated expertise in evidence-based therapeutic techniques for mood disorders.
  • Solid understanding of psychopharmacology to facilitate integrated treatment with medical professionals.
  • A proven track record in delivering effective outcomes in the treatment of mood disorders.

Preferred Experience
  • A minimum of three years of post-graduate experience working directly within mental health counseling.
  • Prior experience working in a hospice or palliative care setting is highly desirable.
