Key Responsibilities
  • Conduct thorough clinical assessments remotely to evaluate the emotional and psychosocial needs of patients receiving hospice care.
  • Develop individualized and culturally-sensitive treatment plans that reflect the unique needs of each patient and their family, aligning with their goals for care.
  • Implement effective solution-based interventions, leveraging evidence-based practices to address issues such as grief, anxiety, and depression.
  • Maintain clear and concise clinical documentation in a timely manner, using our electronic health record system to ensure compliance and continuity of care.
  • Collaborate with a diverse team of healthcare professionals, including nurses, doctors, and chaplains, to coordinate comprehensive care for our patients.
  • Educate and empower patients and families by providing resources and support, helping them navigate the complexities of hospice care.
  • Stay informed about current trends in mental health and hospice care to enhance service delivery and patient outcomes.
Patient Population / Client Focus

In this role, you will work primarily with terminally ill patients and their families, providing emotional support as they navigate the challenges of end-of-life issues. Understanding the delicate nature of these circumstances, you will focus on fostering a safe space where patients can express their fears, desires, and feelings, while guiding their families through the process of grief and loss.

Qualifications
  • A Bachelor’s Degree in Social Work from an accredited institution along with a valid LCSW license in the state of New York.
  • Demonstrated knowledge of solution-focused psychotherapy modalities and psychosocial assessment techniques.
  • Strong interpersonal and communication skills, with the ability to engage empathetically with patients and families in often challenging emotional contexts.
  • Ability to work independently in a remote environment while managing time effectively and staying organized.
Preferred Experience
  • At least two years of clinical experience, preferably in hospice, palliative care, or similar mental health settings.
  • Experience using electronic health record systems and telehealth platforms effectively.
  • Familiarity with interdisciplinary collaboration approaches in a healthcare setting.
