As a Licensed Home Care Services Agency (LHCSA), we are dedicated to supporting both our clients and our caregivers with clinical excellence and heartfelt service.
Position Overview
We are seeking a dedicated Part part-time registered Nurse (RN) to join our team in New York State to support our growing client base. This position would work under the supervision of the Director of Patient Services This is an exciting opportunity for an RN who is passionate about home care and enjoys working independently in the field.
Key Responsibilities
  • Conduct initial RN assessments and evaluations for new clients in accordance with LHCSA and NYS DOH guidelines
  • Complete required reassessments every 6 months or as needed
  • Create and update individualized care plans tailored to each client's needs
  • Supervise Home Health Aides (HHAs) in the field, including performing on-site visits and evaluations
  • Ensure compliance with all clinical documentation and regulatory requirements
  • Collaborate with the care team (Care Coordinators, Scheduling, and Family Members) to ensure high-quality care delivery
  • Provide clinical guidance and oversight to field staff
  • Assist in hiring and training of HHA/PCAs

Qualifications
  • Current NYS Registered Nurse (RN) license in good standing
  • Minimum 1 year of experience in home care, community health, or related field
  • Knowledge of LHCSA regulations and NYS DOH home care standards preferred
  • Strong assessment, communication, and organizational skills
  • Ability to work independently and travel locally to client homes
  • Comfortable with electronic documentation and use of mobile devices
  • Must have a valid driver's license and reliable transportation to visit clients in the Pittsford and greater Monroe County area.
Hourly Wage: $40/hour
