Key Responsibilities
  • Conduct virtual therapy sessions with both individuals and groups, ensuring effective communication and a supportive environment.
  • Develop, implement, and regularly update customized treatment plans that align with each client's unique needs, particularly those related to hospice and end-of-life care.
  • Work closely with interdisciplinary teamsโ€”including nurses, doctors, and other social workersโ€”to enhance integrated care strategies and improve overall client outcomes.
  • Maintain meticulous documentation adhering to HIPAA regulations and organizational standards, ensuring compliance and accountability.
  • Engage in regular supervision and team meetings to discuss case management, share insights, and refine therapeutic approaches based on client feedback and progress.
Patient Population / Client Focus

Your primary focus will be on adults and families navigating the complexities of hospice care. Many of our clients face profound emotional challenges related to grief, loss, and end-of-life transitions. Qualifications
  • Masterโ€™s degree in Social Work from an accredited program.
  • Current and active LICSW license to practice in Massachusetts.
  • A minimum of two years of post-licensure experience in a clinical setting, preferably with a focus on end-of-life issues and hospice care.
  • Demonstrated strong assessment and treatment planning skills, with a commitment to delivering ethical, responsive, and culturally sensitive care.
