Key Responsibilities
  • Conduct virtual therapy sessions for individuals and families dealing with grief, loss, and end-of-life issues, employing evidence-based therapeutic techniques.
  • Develop and implement personalized treatment plans that address the emotional, psychological, and social aspects of patientsโ€™ experiences, ensuring a holistic approach to their care.
  • Engage in regular consultations with physicians, nurses, and other healthcare providers to ensure an integrated approach to patient care, facilitating continuity and coordination throughout the hospice experience.
  • Maintain thorough and confidential patient records in compliance with HIPAA regulations, ensuring that all documentation accurately reflects patient interactions and treatment progress.
  • Provide education to patients and families regarding available resources, coping strategies, and support services tailored to enhance their overall well-being during this challenging time.
    • Patient Population / Client Focus

      The primary focus of this role is on patients receiving hospice care, who may be dealing with terminal illnesses or significant life transitions. This integral position requires sensitivity and emotional intelligence, as you will assist individuals and their families navigating the complexities of end-of-life experiences.

      Qualifications
      • Masterโ€™s degree in Social Work from an accredited institution.
      • Active Licensed Independent Clinical Social Worker (LICSW) license in the state of Massachusetts.
      • A minimum of two years of clinical experience post-licensure, preferably within a healthcare or hospice setting.
      • Strong skills in assessment, diagnosis, and treatment planning, with a solid understanding of mental health challenges of terminally ill patients.
      • Excellent communication skills, with the ability to navigate sensitive conversations and promote a culturally competent environment.
      Preferred Experience
      • Experience working within a hospice or palliative care environment is highly desirable.
      • Familiarity with telehealth platforms and digital communication tools.
