• Conduct comprehensive assessments to understand clients' psychological and social needs, allowing the development of individualized treatment plans.
• Utilize evidence-based practices and interventions, such as cognitive behavioral therapy and solution-focused therapy, to help clients cope with grief, anxiety, and depression.
• Maintain meticulous clinical records and documentation in compliance with HIPAA regulations while ensuring confidentiality and security of patient information.
• Collaborate with a multidisciplinary treatment team, including physicians, nurses, and other therapists, to provide coordinated and comprehensive care.
• Educate and empower patients and their families about the hospice services offered, resources available, and how to navigate their emotional journey.
• Continuously stay informed about developments in mental health care and hospice-specific practices to enhance service delivery and patient outcomes.

This role focuses on a distinctive patient population consisting of individuals with terminal illnesses and their families. You will work with a diverse array of clients, tailoring your approach to meet the unique needs and cultural backgrounds of each family. Providing emotional and psychological support during this sensitive time requires not only clinical skills but also empathy, patience, and the ability to form trusting relationships.

• A Master's Degree in Social Work from an accredited institution is required.
• Current and valid LICSW certification in Massachusetts.
• A minimum of 2 years of post-graduate clinical experience in a mental health or hospice setting.
• Exceptional communication and interpersonal skills, with an emphasis on active listening and compassion.
• Effective problem-solving abilities and critical thinking, particularly in high-stress situations.
• A team-oriented personality that thrives in collaborative environments.

• Experience working with hospice patients and families or similar populations dealing with grief and loss.
• Familiarity with trauma-informed care practices and holistic treatment methodologies.
• Prior practice in using telehealth platforms and digital health tools to engage with clients.
