Position Overview

Gotham Enterprises Ltd is seeking a dedicated and compassionate Remote Licensed Clinical Social Worker (LCSW) to join our team serving a hospice center in Albany, New York. Work Environment

This position is fully remote, providing the flexibility to manage your schedule while remaining committed to your patients' needs. Although this role is conducted remotely, you will be an essential part of a collaborative interdisciplinary team, including nurses, physicians, and chaplains, who work together to ensure holistic care for our patients. Regular virtual team meetings will help maintain communication and foster a supportive team culture.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to evaluate clients' mental health status, understanding their unique challenges, strengths, and available resources.
  • Provide individualized therapy tailored to patients and their families, using evidence-based therapies such as cognitive-behavioral therapy, solution-focused therapy, and grief counseling.
  • Develop and implement personalized treatment plans that outline specific goals and interventions designed to accommodate the individual needs and circumstances of each client.
  • Work collaboratively with an interdisciplinary team, consulting with healthcare professionals such as psychiatrists and nurses to ensure a cohesive approach to patient-centered care.
  • Facilitate supportive group therapy sessions and psychoeducational classes aimed at equipping families with coping strategies and emotional support.
  • Offer crisis intervention and proactive risk assessments to identify and mitigate potential mental health crises.
  • Maintain meticulous documentation of patient interactions, treatment plans, and progress in compliance with ethical, legal, and organizational standards.
Patient Population / Client Focus

The primary focus of this role is on patients receiving end-of-life care in a hospice setting, which necessitates a high degree of sensitivity and understanding of the unique emotional and psychological needs associated with terminal illnesses. This role involves working closely with patients, their families, and caregivers to provide support, facilitate communication, and navigate the challenges of grief and loss.

Qualifications
  • A Master’s degree in Social Work from an accredited program is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A minimum of two years of experience in mental health, hospice, or outpatient care settings, with a demonstrated ability to work effectively with diverse populations.
  • Strong knowledge of evidence-based therapeutic modalities and a passion for applying them in real-world contexts.
  • Excellent interpersonal and communication skills, with a strong ability to empathize with patients and their families during emotionally charged situations.
Preferred Experience

Experience in palliative care settings or working specifically with grief and bereavement support is preferred. Proficiency in interdisciplinary collaboration and knowledge of hospice regulations can provide an additional advantage.

Compensation and Benefits

The compensation for this role ranges from USD 80,000 to 100,000 annually, reflecting experience and qualifications.
