Active Health & Hospice is seeking a compassionate and dedicated Licensed Independent Clinical Social Worker (LICSW) to join our dynamic team on a full-time remote basis. In this role, you will be an integral part of providing mental health support to students in a school setting, helping them navigate their emotional and psychological challenges. Your expertise will enable you to develop tailored treatment plans and implement effective therapeutic interventions, significantly contributing to the well-being of our clients.

Key Responsibilities
  • Conduct comprehensive mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments to understand the unique needs of each student.
  • Collaborate with students and their families to develop individualized treatment plans, incorporating specific goals, cultural considerations, and preferred therapeutic approaches.
  • Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), across various settings including individual, couple, family, and group therapies.
  • Provide crisis intervention services to students in acute distress, ensuring swift coordination with other healthcare professionals to deliver continuity of care.
  • Maintain accurate and up-to-date clinical documentation, including progress notes, treatment plans, and discharge summaries as per organizational policies and regulatory compliance.
  • Deliver psychoeducation to students and their families, equipping them with essential knowledge and coping strategies for effective management of mental health conditions.
  • Work closely with a multidisciplinary team, comprising psychiatrists, psychologists, nurses, and school counselors, to coordinate and enhance comprehensive care for patients.
  • Evaluate treatment effectiveness through regular outcome assessments and adjust treatment plans accordingly to meet evolving patient needs.
  • Stay informed on current research, emerging trends, and best practices in mental health to continuously enhance clinical practice.
  • Adhere strictly to ethical guidelines and maintain confidentiality of all patient information.
Patient Population / Client Focus

This role focuses on supporting students from diverse backgrounds, including those facing socio-economic challenges, learning disabilities, and mental health issues. Your work will have a profound impact on their emotional resilience and overall academic success.

Qualifications
  • A Master's degree in Social Work (MSW) is required for this role.
  • Active State licensure as a Licensed Independent Clinical Social Worker (LICSW) is mandatory.

  • Exceptional interpersonal and communication skills, with demonstrated ability to connect with students and their families from varied cultural backgrounds.
Preferred Experience
  • Experience working in a school setting or with children and adolescents in a mental health context is highly desirable.
