Key Responsibilities

  • Document all patient interactions and care activities in the electronic medical record system within 48 hours to maintain accurate and up-to-date patient information.
  • Develop, implement, and monitor individualized care plans that align with the patient's needs and link them to necessary services, while ensuring effective communication among all treatment team members.
  • Safeguard patient privacy and confidentiality in all communications, documentation, and data handling.
  • Engage in supervision, case conferences, and departmental meetings to enhance team collaboration and quality improvement practices.
  • Provide referrals for prescribed psychiatric medications and connect patients with community resources that support their overall goals and interests.
  • Return calls and follow up with patients, families, and other providers to address concerns or schedule appointments.
Patient Population / Client Focus

Qualifications
