• Conduct comprehensive psychosocial assessments of seniors to identify emotional, mental, and social needs, ensuring a personalized approach to care.
• Collaborate with clients and their families to formulate tailored care plans that align with individual goals and preferences, focusing on interventions that can alleviate anxiety and depression.
• Provide counseling and emotional support to clients as they navigate life transitions and challenges, including bereavement, changes in health status, and family dynamics.
• Advocate for clients’ rights to access relevant community resources, liaising with social service agencies to facilitate appropriate support and services.
• Engage with interdisciplinary teams comprising healthcare professionals, nurses, and caregivers to streamline care coordination and improve health outcomes.
• Carry out regular check-ins to monitor clients’ well-being, assessing living conditions and addressing safety concerns promptly.
• Facilitate family communication, mediate disputes, and enhance the quality of relationships among family members involved in the senior client's care process.
• Educate clients and their families on community resources, support groups, and programs that foster social engagement and enhance quality of life.
• Maintain meticulous documentation of client assessments, individualized care plans, progress notes, and any relevant interactions, ensuring compliance with legal and ethical standards.
• Stay updated with current best practices in social work, policies affecting the aging population, and relevant regulations, ensuring adherence to ethical standards.

This position emphasizes work with seniors who often face complex psychosocial challenges, including mental health issues, social isolation, and adapting to significant life changes. A deep understanding of the unique needs of this demographic is essential, as well as a commitment to delivering empathetic and culturally responsive care.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work capacity, ideally working within a community health setting or geriatric care.
• Extensive knowledge of social work principles, best practices, and resources relevant to aging populations.
• Exceptional assessment, communication, and problem-solving skills.
• A strong capacity for empathy and the ability to forge strong relationships with clients and families.
• Proficiency in electronic health records (EHR) systems and standard computer applications.

• Experience within a community health clinic or geriatric care setting is highly desirable.
• Familiarity with local community resources and advocacy services available for seniors.
• Background in crisis intervention and conflict resolution within family dynamics.
