Key Responsibilities
  • Conduct thorough assessments and re-evaluations to effectively meet the diverse mental health needs of patients.
  • Maintain open lines of communication among team members, clients, their families, and other stakeholders regarding patient progress and care plans.
  • Develop and share a tentative schedule detailing upcoming appointments, home visits, and observations to facilitate seamless progress monitoring.
  • Coordinate and execute home visits for in-home assessments, ensuring compliance with program requirements and wellness initiatives.
  • Oversee the implementation of treatment plans and ensure equitable and effective delivery of services to all clients.
  • Actively participate in Interdisciplinary Team Meetings and provide input on clinical discussions and treatment strategies.
  • Prepare and maintain accurate monthly and quarterly progress notes for documentation and reporting purposes.
  • Lead training sessions aimed at certifying staff in the effective implementation of treatment plans and necessary documentation processes.
  • Conduct follow-up sessions, including interviews and home visits, to assess and support ongoing patient needs.
  • Serve as the public liaison for mental health initiatives, collaborating with community organizations and resources to drive developmental goals.
  • Assist in creating, updating, and refining training materials to enhance program effectiveness and staff competency.
Patient Population / Client Focus

This role will involve providing care to a diverse array of patients, including both adults and youth. You will work with individuals experiencing a range of mental health challenges, from anxiety and depression to trauma and substance use disorders. Our community-oriented approach aims to reach those who may have previously faced barriers to accessing necessary mental health services.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • An active and unrestricted Professional Counselor (LPC) license in the state of New Jersey is mandatory.
  • A minimum of 2 years of counseling experience in clinical settings is required, demonstrating the ability to address complex client needs.
Preferred Experience
  • Experience or specialization in family therapy, couples therapy, trauma-informed care, or substance misuse counseling is highly preferred.
  • A robust understanding of legal and ethical standards within the mental health profession is essential.
  • Excellence in interpersonal and communication skills to build rapport with a diverse clientele effectively.
  • Previous experience working collaboratively within healthcare teams is an advantage.
  • Competence in using Electronic Medical Record (EMR) software and related digital tools is preferred for efficient documentation and patient management.
