Key Responsibilities

  • Collaborate with the Social Work Manager to develop and implement individualized treatment plans, utilizing evidence-based practices in both individual and group settings.
  • Provide compassionate and ethical outpatient mental health therapy to patients facing various challenges, including those related to mental and physical illnesses.
  • Document social work activities comprehensively and timely within patients' medical records, adhering to departmental guidelines and standards.
  • Educate patients and families on medical and social work recommendations, facilitating understanding and adherence to treatment protocols.
  • Act as an advocate for patients and their families within the community, coordinating services and resources to enhance their social support systems.
  • Remain current on community resources and training, ensuring you address the specific needs of those served in the hospice context.
  • Support care coordination with medical and support staff, fostering an integrated care approach that enhances treatment outcomes.
Patient Population / Client Focus

Your primary focus will be on patients experiencing end-of-life challenges, their families, and the psychosocial issues that may arise during such a profound time. You will be instrumental in ensuring that patients receive holistic care that honors their values and preferences while addressing their psychological needs. With your expertise, you will help families navigate the complexities of grief and support their emotional well-being.

Qualifications
  • A Master’s Degree in Social Work from an accredited institution.
  • A current Licensed Clinical Social Worker (LCSW) license from the State Board of Social Work Licensure.
  • Knowledge and skills necessary to provide care that respects the aging process, human development stages, and cultural considerations.
  • A minimum of one year of clinical experience, preferably in a health or mental health setting, with a focus on populations similar to those served in hospice care.
Preferred Experience
  • Experience working in hospice, palliative care, or similar settings where end-of-life issues are a focal point.
  • Training in trauma-informed care and competency in addressing complex grief and bereavement issues.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 85,000 to 100,000, aligned with experience and educational background.
