Key Responsibilities

• Conduct comprehensive bio-psychosocial evaluations to diagnose psychosocial problems and develop personalized treatment plans that prioritize individual patient needs.
• Collaborate closely with a multidisciplinary team to employ evidence-based treatment modalities and integrated care approaches, ensuring comprehensive support for each patient.
• Maintain detailed documentation of social work activity and patient interactions, adhering to departmental quality standards and legal regulations.
• Provide education and guidance to patients and their families, helping them to understand and engage with medical and social work recommendations for optimal health restoration.
• Manage case-related statistics and reports to enhance the quality and impact of services delivered, contributing to the organization’s goal of improving patient care outcomes.

Patient Population / Client Focus

Your role will focus on a diverse patient population, addressing their unique mental health needs with cultural sensitivity and an understanding of individual developmental stages. Your expertise will be essential in guiding patients toward achieving their highest level of psychosocial functioning.

Qualifications

• A Master’s Degree in Social Work from an accredited institution, complemented by a current LCSW license issued by the State Board of Social Work Licensure.
• Demonstrated experience and competency in providing psychological support tailored to the specific needs of the populations you serve.
• Solid understanding of the aging processes, human development, and cultural patterns that affect care delivery and patient interactions.
