• Conduct in-depth psychosocial assessments to identify the emotional, social, and mental health needs of seniors. Utilize standardized assessment tools and frameworks to ensure comprehensive evaluation.
• Develop and implement personalized care plans for each client in collaboration with their families, ensuring that strategies align with their specific goals and preferences.
• Provide individual and family counseling that empowers clients to navigate the challenges associated with aging, including health transitions, loss, and emotional distress.
• Serve as an advocate for clients by connecting them to essential community resources, facilitating access to healthcare services, and rights protection.
• Maintain accurate and up-to-date client records through electronic health record systems, ensuring compliance with state regulations and organizational policies.
• Engage in regular supervision and consultative meetings with fellow team members to enhance practice and discuss client progress.

As a Licensed Clinical Social Worker at Maple Valley Counseling Center, your primary focus will be on seniors, a demographic facing unique emotional and social challenges. You will work closely with individuals experiencing age-related conditions, providing them with the necessary tools and emotional support to thrive. • Master’s degree in Social Work (MSW) from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.
• Minimum of two years of clinical experience in a social work setting, preferably with a focus on geriatric populations.
• Proficient in assessment, counseling, and case management techniques, with a solid understanding of social work ethics and confidentiality requirements.
• Strong communication, empathy, and problem-solving skills essential for effective client interactions.

• Experience working within community health clinics or related organizations, especially with vulnerable populations.
• Familiarity with mental health issues affecting older adults and knowledge of intervention strategies relevant to this client group.
• Demonstrated ability to work autonomously while contributing to a collaborative team environment.
