Work Environment

This position is designated as remote, allowing you the flexibility to work from the comfort of your own home while still being connected to a passionate team committed to serving those in need. As a member of our dynamic hospice center, you are expected to engage in virtual meetings, collaborative projects, and face-to-face community outreach initiatives that strengthen our impact. Key Responsibilities
  • Provide individual and group therapy sessions to clients, addressing their various psychological and emotional needs.
  • Develop and implement personalized care plans that include evidence-based practices, engaging clients and their families in the therapeutic process.
  • Conduct initial assessments and develop integrated care plans that promote access to resources and support networks tailored to each clientโ€™s situation.
  • Maintain accurate and confidential client records that adhere to ethical and professional standards, facilitating effective communication within the clinical team.
  • Collaborate with other healthcare providers, including nurses and physicians, to ensure a holistic approach to client care, navigating any barriers to treatment.
  • Participate in ongoing training and professional development to enhance your therapeutic skills and knowledge of best practices in social work.
  • Identify risk factors or unsafe situations and report them promptly, ensuring a safe environment for all clients and staff.
