Work Environment

This position is designated as remote, allowing you the flexibility to work from the comfort of your own home while still being connected to a passionate team committed to serving those in need. As a member of our dynamic hospice center, you are expected to engage in virtual meetings, collaborative projects, and face-to-face community outreach initiatives that strengthen our impact. You will be equipped with all necessary tools and resources to deliver exceptional care from a distance.

Key Responsibilities

• Provide individual and group therapy sessions to clients, addressing their various psychological and emotional needs.
• Develop and implement personalized care plans that include evidence-based practices, engaging clients and their families in the therapeutic process.
• Conduct initial assessments and develop integrated care plans that promote access to resources and support networks tailored to each client’s situation.
• Maintain accurate and confidential client records that adhere to ethical and professional standards, facilitating effective communication within the clinical team.
• Collaborate with other healthcare providers, including nurses and physicians, to ensure a holistic approach to client care, navigating any barriers to treatment.
• Participate in ongoing training and professional development to enhance your therapeutic skills and knowledge of best practices in social work.
• Identify risk factors or unsafe situations and report them promptly, ensuring a safe environment for all clients and staff.
• Utilize electronic health records (EHR) to document care, treatment plans, and progress, ensuring compliance with all healthcare regulations.

Patient Population / Client Focus

As a Licensed Clinical Social Worker at Phoenix Behavioral Hub, you will primarily serve clients dealing with major mental illness and severe emotional disorders. Our client population may include individuals experiencing various challenges, such as depression, anxiety, trauma, and chronic illness. With a commitment to inclusivity, you will work with diverse individuals and families, tailoring approaches to honor their unique backgrounds, beliefs, and cultural perspectives.

Qualifications

• A Bachelor’s Degree in Social Work, Psychology, or a related field is required; an LCSW is essential.
• Strong understanding of mental health diagnostics and treatment methodologies.
• Excellent communication and interpersonal skills, with a demonstrated ability to build rapport with clients.
• Proficient in technology and familiar with electronic health records (EHR) systems.
• Ability to work independently and as part of a collaborative team.

Preferred Experience

• Experience working in a hospice or palliative care setting is highly desirable.
• Familiarity with crisis intervention and trauma-informed care approaches.
• Previous work with underserved populations and an understanding of social determinants of health.

Compensation and Benefits

The compensation for this position ranges from $70,000 to $85,000 annually, commensurate with qualifications and experience.
