• Collaborate with a multidisciplinary team of physicians, nurses, and other healthcare professionals to develop individualized care plans for hospice patients.
• Conduct thorough clinical assessments and utilize evidence-based practices to inform treatment strategies that address clients' psychological and social needs.
• Lead case planning sessions, working closely with clients and their families to set achievable goals and outcomes, ensuring all parties are engaged in the process.
• Educate clients and their families about available mental health resources, including community services, coping strategies, and interventions to promote overall well-being.
• Stay abreast of the latest advancements in mental health care to enhance your practice and provide the most effective support.

Your primary focus will be on older adults facing terminal illnesses and their families, guiding them through the complex emotions associated with end-of-life care. You will address grief, anxiety, and depression through empathetic counseling and therapeutic techniques tailored to the unique challenges presented by this population.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Valid and active LCSW license in the state of Nevada.
• Demonstrated experience working with older adults, particularly in hospice or end-of-life settings.
• Strong proficiency in conducting clinical assessments and developing innovative treatment plans.
• Excellent interpersonal skills and a demonstrated ability to collaborate with diverse teams.

• At least 2 years of experience in a social work or counseling role, ideally within a hospice or healthcare environment.
• Familiarity with telehealth systems and electronic health records (EHR) to facilitate remote client interactions.
• Understanding of hospice policies, practices, and relevant regulations to ensure compliance and quality care.
