• Conduct thorough assessments of patients and their families to identify emotional, psychological, and social needs related to their hospice experience.
• Develop individualized care plans in collaboration with interdisciplinary teams, ensuring that each plan aligns with patient goals, preferences, and values.
• Implement evidence-based therapeutic interventions, providing counseling and support to assist patients and their families in coping with grief, loss, and end-of-life issues.
• Maintain accurate and confidential documentation of patient interactions, treatment plans, and progress notes in accordance with HIPAA regulations.
• Collaborate with medical staff, nurses, and other professionals to ensure comprehensive, holistic care for patients, sharing insights and strategies to enhance overall patient well-being.
• Stay abreast of industry best practices and advancements in social work by engaging in professional development and participating in relevant training.

As a social worker within our hospice center, you will work with a diverse population of patients and families navigating the complexities of terminal illness. Your role will involve addressing their psychological and social concerns while providing support during one of the most challenging times in their lives. You will engage with families to help them prepare for and cope with grief and bereavement, ensuring they feel heard, understood, and supported throughout the entire process.

• A Master’s degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Exceptional communication skills, with the ability to empathize and build rapport with a vulnerable patient population.
• Demonstrated ability to work collaboratively in a team environment and contribute to a shared vision for high-quality care.

• A minimum of 2 years of experience in clinical social work, particularly within hospice, palliative, or end-of-life care settings.
• Familiarity with grief counseling and support techniques, as well as experience working with patients facing chronic illnesses.
• Proficient in using electronic health records (EHR) systems to document patient interactions and care strategies efficiently.
