Active Minds Behavioral Counseling is seeking a dedicated and compassionate Clinical Social Worker to join our team in a fully remote setting, focusing on hospice care. Key Responsibilities
  • Conduct thorough psychosocial assessments to evaluate clients' emotional and mental health needs, identifying strengths and resources to build upon during treatment.
  • Provide individual and family therapy, employing evidence-based therapeutic modalities, including Cognitive Behavioral Therapy (CBT), solution-focused therapy, and trauma-informed care tailored to each client's unique experiences.
  • Create and implement personalized treatment plans that clearly outline achievable goals, interventions, and measurable outcomes for each client.
  • Collaborate with interdisciplinary teams, including physicians, psychiatrists, and other healthcare professionals, to coordinate comprehensive care for clients and assure their needs are met.
  • Facilitate group therapy sessions and psychoeducational workshops that empower clients to navigate their emotional journey effectively.
  • Respond promptly to crises, conducting risk assessments and developing safety plans as necessary, while maintaining a compassionate and supportive approach.
  • Ensure accurate and timely documentation of client interactions, treatment plans, and progress in compliance with ethical and legal standards.
  • Participate in regular team meetings to discuss client progress and ongoing treatment strategies, contributing your unique insights and expertise.
Patient Population / Client Focus

This role primarily serves individuals and families receiving hospice care, many of whom are dealing with grief, anxiety, and existential distress. You will be integral in helping clients cope with complex emotions surrounding terminal diagnoses and support their families as they navigate these challenging times. Your expertise in managing trauma-related issues and providing compassionate support will be invaluable in enhancing their quality of life.

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited program is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
  • A minimum of two years of experience in a mental health or outpatient setting, preferably in hospice or palliative care.
  • Strong command of evidence-based therapeutic approaches and familiarity with various interventions relevant to diverse client needs.
  • Excellent communication, interpersonal, and assessment skills, with an emphasis on empathy and active listening.
Preferred Experience

Compensation and Benefits
