As a Hospice Triage Registered Nurse, You will: 

  • Answer hospice calls during nights, weekends, holidays, and emergencies (severe weather for example).  
  • Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.  
  • Talk with patients and caregivers to assess needs and provide support over the phone.  
  • Communicate with doctors about patient updates and get new care orders if needed.  
  • Give instructions and guidance based on each patient’s care plan. 
  • Coordinate after-hours needs such as equipment, supplies, and medications.  
  • Work with facility staff to support their patients during off-hours.  
  • Document all calls, care updates, and decisions clearly and accurately.  
  • Ensure all care and procedures follow state and federal rules.
