Key Responsibilities
  • Conduct comprehensive assessments of patients to tailor treatment plans to their specific needs, ensuring ongoing evaluations and adjustments as necessary.
  • Facilitate clear communication among supervisors, care staff, family members, and other relevant parties to coordinate patient progress and any necessary interventions.
  • Prepare and share tentative schedules for patient appointments, home visits, and observations to ensure effective monitoring and follow-up care.
  • Coordinate home visits to carry out assessments in the patientโ€™s environment, including chart reviews, interviews with staff, and data analysis to inform treatment strategies.
  • Ensure that implemented programs are effective, compliant with healthcare regulations, and appropriately documented.
  • Participate in Interdisciplinary Team Meetings and contribute to discussions regarding patient care and program improvements.
  • Complete and submit monthly and quarterly progress notes that accurately reflect the patientโ€™s progress and adjustments to treatment plans.
  • Organize and deliver staff training initiatives, ensuring team members are equipped to adhere to program standards and documentation requirements.
  • Conduct follow-up interviews and home visits to provide ongoing support and adjust care plans as necessary.
  • Maintain organized and confidential patient records to facilitate easy access for authorized personnel.
  • Act as a liaison within the community and with other mental health professionals to advocate for program development and patient advocacy.
Patient Population / Client Focus

You will work with a diverse patient population, including both adults and youth, with various mental health challenges. This role emphasizes collaboration with clients and their support networks to ensure comprehensive care and successful treatment outcomes. By providing personalized support, you will empower clients to confront their challenges, enhance their skills, and improve their overall quality of life.

Qualifications
  • A Masterโ€™s or Doctoral degree in Counseling, Psychology, or a related field.
  • Active and unrestricted license as a Professional Counselor (LPC) in Georgia.
  • A minimum of 2 years of counseling experience in a clinical setting is required.
  • Strong understanding of legal and ethical standards in mental health practice.
