Crystal Psychotherapy is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. In your role as a Licensed Clinical Social Worker, you will be responsible for:

• Conducting comprehensive assessments to develop tailored treatment plans for hospice patients dealing with end-of-life issues.
• Providing individual and family therapy sessions, focusing on grief and loss, emotional distress, and coping strategies.
• Facilitating communication between patients, families, and caregivers to ensure a holistic support system and resource accessibility.
• Assisting families in managing complex emotions associated with terminal illnesses, including anticipatory grief and feelings of loss.
• Collaborating with medical staff to integrate behavioral health into the overall care plan, ensuring that emotional and psychological needs are met.
• Maintaining detailed and accurate documentation within electronic medical records (EMR), contributing to comprehensive care coordination.
• Participating in weekly clinical meetings to discuss case progress and share insights with fellow team members.

This position will primarily focus on working with individuals who are in hospice care, along with their families. You will address a variety of issues, including:

• Adjustment to terminal diagnoses and end-of-life planning.
• Coping strategies for patients facing debilitating health challenges and the emotional toll of their conditions.
• Support for families dealing with anticipatory grief and loss.
• Encouraging communication within families to address unresolved conflicts, enhancing relational ties during critical moments.
• Fostering resilience and acceptance among patients and their loved ones as they navigate the complexities of end-of-life care.

The ideal candidate will possess the following qualifications:

• A Bachelor's degree in Social Work (BSW) or a related field is required.
• An active and unrestricted LCSW license in the state of Colorado.
• A minimum of 2 years of clinical experience, particularly in hospice, palliative care, or similar settings.
• Strong interpersonal skills and the ability to build rapport quickly with patients and families.
• Compassionate, patient-oriented approach with excellent clinical judgment.
• Proficiency in electronic health record (EHR) systems and telehealth platforms.

While not mandatory, the following experience will be considered advantageous:

• Prior experience working with diverse populations and effectively addressing multicultural factors in treatment.
• Experience in crisis intervention and managing sensitive conversations with families regarding care decisions.
• Familiarity with hospice care protocols, interdisciplinary team functions, and community resources for referral.

The compensation for this role ranges from USD 70,000 to 85,000 yearly, commensurate with experience and qualifications. In addition to competitive salary, we offer:
