While the position is primarily remote, you will also engage with interdisciplinary teams, involving nurses, physicians, and support staff, to coordinate care for our patients. • Conduct comprehensive mental health assessments for patients, taking into consideration their personal histories, emotional needs, and end-of-life concerns.
• Develop and implement tailored treatment plans that focus on addressing mental health issues, enhancing emotional well-being, and providing support for family members.
• Facilitate evidence-based therapeutic interventions such as psychotherapeutic counseling, grief support, and adjustment counseling, specifically designed for hospice patients and their families.
• Provide individual and group therapy sessions aimed at fostering personal growth, emotional regulation, and connection with support systems.
• Actively collaborate with fellow healthcare professionals including counseling staff, medical providers, and spiritual care team members to ensure a holistic and integrated approach to patient care.
• Offer crisis intervention support and guidance during mental health emergencies, employing de-escalation techniques and making appropriate referrals for additional care as needed.
• Deliver psychoeducation sessions to patients and their families about coping strategies, available resources, and the emotional processes of grief and loss.
• Ensure meticulous documentation of assessments, treatment progress notes, and care plans in line with legal and ethical standards through electronic health record (EHR) systems.
• Participate in ongoing professional development opportunities and contribute to multidisciplinary team discussions, fostering a culture of continuous improvement.

Your primary focus will be on patients residing in our hospice center who are facing life-limiting conditions. This includes providing support to diverse individuals and families who are navigating complex emotional landscapes, often facing grief, anxiety, and existential concerns. It is essential to approach each case with sensitivity and respect, ensuring that the care provided honors the unique histories and wishes of each patient.

• A Master's degree in Social Work from an accredited institution is required.
• Valid Licensed Clinical Social Worker (LCSW) licensure in the state of Colorado.
• A minimum of two years of post-licensure experience, preferably in mental health, hospice, or palliative care settings.
• Deep understanding of mental health disorders, evidence-based modalities, and trauma-informed approaches.
• Competence in conducting mental health assessments and crafting effective treatment plans tailored for geriatric populations.
• Experience in delivering therapeutic interventions within diverse groups and one-on-one settings.
• Strong crisis management skills alongside the ability to maintain composure in high-stress environments.
• Excellent communication and interpersonal skills, coupled with the ability to connect with clients from various backgrounds.

• Experience working in hospice or end-of-life care settings.
• Familiarity with electronic health record (EHR) systems to document patient interactions and treatment plans effectively.
• Prior experience in providing grief counseling and supporting families during bereavement.
