As a Licensed Mental Health Counselor, you will play a critical role in patient care through a variety of essential tasks, including:

• Reviewing patient health histories to prepare for initial evaluations and assessments.
• Developing, implementing, and periodically reevaluating individualized mental health care plans tailored to the unique needs of each patient.
• Conducting in-depth interviews with patients and their families to identify educational needs, establish treatment goals, and formulate comprehensive care plans.
• Evaluating patient responses to educational efforts and adjusting care plans as necessary to maximize treatment effectiveness.
• Collaborating with medical professionals and interdisciplinary team members to coordinate and communicate patient care plans.
• Maintaining strict confidentiality and adhering to HIPAA regulations while documenting patient interactions and care plans diligently.
• Conducting multiple Healthy Minds workshops aimed at promoting mental health awareness and education.
• Making appropriate referrals to other healthcare specialists based on assessments and patient needs.

You will primarily work with individuals dealing with various mental health challenges, including those undergoing treatment for cardiac conditions. Understanding the unique intersection of mental health and physical health will be essential in providing comprehensive care. Your efforts will not only help patients navigate their mental health journeys but also enhance their overall well-being.

To be successful in this role, candidates must meet the following requirements:

• Current licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Psychologist.
• A Bachelor's degree in Psychology, Social Work, Counseling, or a related field is required.

While not mandatory, candidates with previous experience working in community health settings or those who have conducted group therapy or educational workshops will have a distinct advantage. Familiarity with telehealth practices and digital record-keeping systems will also be beneficial.
