• Conduct thorough assessments and ongoing evaluations to identify the specific mental health needs of each patient served.
• Maintain transparent communication with patients, supervisors, direct care staff, family members, and guardians to discuss progress and treatment plans.
• Prepare and synchronize a schedule for appointments, home visits, and various monitoring activities to ensure comprehensive care.
• Organize and facilitate home visits and evaluations to achieve thorough understanding and effective treatment strategies for patients.
• Ensure that all program plans are implemented effectively, and remain aligned with best practices and patient needs.
• Engage actively in Interdisciplinary Team Meetings and contribute to other relevant meetings to share insights and develop cohesive treatment approaches.
• Document monthly and quarterly progress notes to track patient progress and treatment effectiveness.
• Oversee training activities for staff members to ensure they are equipped to implement program goals and maintain proper documentation.
• Provide ongoing follow-up through interviews, home visits, and observations, fostering continued support in meeting each individual's needs.
• Act as a liaison among the public and mental health communities, assisting with initiatives geared toward developmental goals and community outreach efforts.

You will work with a diverse range of clients, including adults and youth facing various challenges such as family issues, trauma, substance misuse, and the spectrum of mental health conditions. Your expertise will be paramount in addressing these issues, providing specialized care that promotes stabilization and recovery in individuals at every level of need.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona is essential.
• A minimum of 2 years' experience in counseling, particularly within a clinical setting, is mandatory.
• Strong grasp of legal and ethical standards relevant to counseling practices is required.
• Exceptional interpersonal skills and the ability to build rapport with a varied clientele are crucial for your success.
• Demonstrated ability to work collaboratively with a healthcare team is essential for optimal patient outcomes.
• Proficiency in using Electronic Medical Records (EMR) software and related technologies to document client interactions effectively.
