• Conduct ongoing assessments tailored to the needs of each patient, ensuring individualized treatment plans that foster their mental health journey.
• Actively communicate with supervisors, team members, and stakeholders regarding individual patient progress, facilitating a cohesive approach to care.
• Develop and provide a preliminary schedule for appointments, home visits, and clinical observations to supervisors, enhancing treatment continuity.
• Coordinate and execute home visits as necessary to complete thorough evaluations, including chart reviews and staff interviews.
• Ensure that the program's treatment plans are effectively implemented and properly monitored to meet individual goals.
• Participate in interdisciplinary team meetings to collaborate on treatment strategies and report patient progress regularly.
• Maintain meticulous records, including monthly and quarterly progress notes, to ensure transparency and accessibility for all concerned parties.
• Serve as a liaison between clients and the public mental health community, advocating for patients' needs and enhancing community engagement.
• Organize training sessions for staff to ensure competency in implementing treatment plans and maintaining accurate documentation.
• Provide follow-up support through interviews and home visits, ensuring sustained engagement with patients post-treatment.

The Licensed Professional Counselor will engage with a diverse clientele, including both adults and youth facing a variety of mental health challenges. Your work will significantly impact individuals coping with trauma, substance misuse, family dynamics, and other interpersonal issues. • Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Possession of an active, unrestricted license as a Professional Counselor (LPC) in New Jersey.
• A minimum of 2 years of relevant counseling experience, particularly in a clinical environment, is essential.
• Specialization, especially in family therapy, couples therapy, trauma, or substance misuse, is particularly valued.
• A strong understanding of ethical and legal standards relevant to counseling and mental health.
• Demonstrated excellent interpersonal and communication skills, with the ability to foster rapport with clients from diverse backgrounds.

• Competence with electronic medical records (EMR) and related technologies to maintain client information accurately.

• A track record of developing client-centered treatment plans based on assessments and client feedback.
