Work Environment

This position offers a unique remote work setting, allowing our counselors the flexibility to create a workspace that suits their needs. Although primarily conducted remotely, there may be opportunities for in-office collaboration within our outpatient facility as needed. Key Responsibilities
  • Conduct thorough assessments and ongoing evaluations for patients, ensuring their mental health needs are accurately addressed.
  • Facilitate effective communication among supervisors, caregivers, and family members to foster a supportive environment for patient growth.
  • Develop and share treatment plans outlining patient appointments, home visits, and observation sessions, actively tracking progress and adjusting strategies as necessary.
  • Organize and conduct home visits for comprehensive in-home evaluations, contributing to our overall understanding of patient circumstances.
  • Ensure that all program strategies are executed with fidelity and adherence to clinical best practices.
  • Participate in Interdisciplinary Team Meetings, sharing insights and collaborating with fellow professionals for holistic care delivery.
  • Maintain accurate and up-to-date documentation, including monthly and quarterly progress notes, to support ongoing treatment plans.
  • Provide training and support to team members, ensuring they are proficient in implementing program plans and managing paperwork effectively.
  • Conduct follow-up interviews and observations to evaluate the effectiveness of interventions and adjust care accordingly.
  • Act as a liaison between our clinic and the community, promoting mental health awareness and ensuring that all individuals have access to necessary services.
Patient Population / Client Focus

The Licensed Professional Counselor will work with a diverse clientele, including children, youth, and adults, facing a range of mental health challenges. Your expertise in family dynamics, substance misuse, or trauma will help you connect with and support these individuals while promoting healthier lifestyles and emotional resilience.

Qualifications
