The Health and Social Services Manager will manage a team of Registered Nurses that supports care of members within the community by completing Face to Face visit assessments and telephonic member care coordination.
 


This is a remote role with the possibility of minimal travel within an assigned territory. If you reside in or within commutable driving to Tarrant County, TX, and surrounding areas, this would be a great opportunity.


If you are located in or within commutable driving distance to NE Dallas, TX, you will have the flexibility to work remotely* as you take on some tough challenges.


Primary Responsibilities:

  • Supports clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating)
  • Engages in case management and coordination of care, supporting RN Nurse Care Coordinator team members in the field
  • Takes lead role in setting direction and participating in or developing new programs
  • Sets team direction, resolves problems, and provides guidance to members of the team
  • Ensure team meets established performance metrics and contractual obligation
  • Adapts departmental plans and priorities to address business and operational challenges
  • Plans, organizes, and oversees RN Nurse Care Coordinator to ensure timely completion of assessments, documentation, and task
  • Oversees the department's daily staffing requirements to meet program standards
  • Implements organizational policies and procedures, reviews work of RN Nurse Care Coordinator to ensure policies and guidelines are appropriately applied
  • Other duties as assigned to support team

Required Qualifications:

  • Current, unrestricted RN license in the State of Texas
  • 3 years of management experience in healthcare OR 3 years of experience as a Field Service Coordinator/ Care Coordinator
  • 1 years of experience with long term care services and support (LTSS)
  • 1 years of Medicare and / or Medicaid experience  
  • Intermediate MS office including Word, Excel, and Outlook with proficient ability to navigate in a Windows environment
  • Reside in or within commutable driving distance from the Dallas, TX
  • Valid driver's license, access to reliable transportation, and the ability to travel up to 25% within assigned territory as needed


Preferred Qualifications:

  • BSN
  • Certified Case Manager (CCM)
  • 5 years of experience in a nursing facility setting, or hospital setting, or case management, or home health care or clinical experience in acute / rehab, HCBS / Level-1 field service coordination
  • Case Management experience  
  • Experience in forming and establishing relationships with state partners
  • Background in managed care - experience working with vulnerable populations who have chronic or complex bio psychosocial needs
  • Bilingual skills
  • Reside in Dallas, TX area

The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
