This remote position allows you to work from the comfort of your home while connecting with patients from various backgrounds through a Hospice Center. Regular virtual team meetings and collaborative tools ensure all staff stay aligned on patient progress and treatment plans.

• Conduct comprehensive psychosocial assessments of patients through virtual interviews, observations, and analysis of medical histories to identify emotional, social, and environmental factors affecting their recovery.
• Deliver personal counseling and support services to patients and their families, addressing their emotional needs and guiding them through periods of adjustment and change.
• Collaborate with rehabilitation teams to create tailored treatment plans that integrate patients’ psychosocial needs into their overall care strategy.
• Facilitate patients' access to community resources, such as support groups and vocational rehabilitation services, ensuring they receive the comprehensive support they need for success.
• Educate patients and their families about post-rehabilitation services, community resources, and the importance of ongoing support networks.
• Assist in discharge planning and effectively manage transitions to home-based or alternative care settings to promote continuity of care.

Our patient population includes individuals facing life-altering events, such as strokes, spinal cord injuries, and extensive orthopedic surgeries. As an LCSW, you will play an essential role in assisting these clients in overcoming the psychosocial hurdles of their conditions, fostering resilience, and promoting their re-engagement with daily life activities.

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated knowledge of adjustment disorders, coping mechanisms, and grief counseling methodologies.
• Excellent communication and interpersonal skills that facilitate meaningful connections with patients and families.

• Prior experience in a rehabilitation or medical setting, particularly within hospice care.
• Understanding of interdisciplinary approaches to patient care, emphasizing collaboration among healthcare team members.
• Experience using electronic health record (EHR) systems to document patient progress and treatment interventions accurately.
