Description

The Azalea Health Analyst serves as a key member of the Population Health and Value-Based Care team, supporting Accountable Care Organization (ACO) initiatives through the administration, optimization, analysis, and reporting of the Azalea Health Population Health platform. This role partners with clinical, operational, quality, care management, and executive stakeholders to leverage data, improve patient outcomes, identify care gaps, support quality performance programs, and maximize success across value-based reimbursement arrangements.

This is a Contract position based out of Philadelphia, PA.

Pay and Benefits

The pay range for this position is $80.00 - $90.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
