MedZed is a leader in delivering value-based, technology-enabled social support to a diverse population of high-cost Medicaid members who have been unreachable through telephonic outreach, disconnected from primary care, and using hospital-based services as their primary point of care.

We combine innovative technology with field-based outreach to find and engage these members. We then apply a model of care designed to reconnect members to primary care, address the health-Related Social Needs (HRSNs) that contribute to their disengagement and create barriers to care, and provide members with the resources, knowledge, and support needed to take greater control of their healthcare.

Our interventions help reduce Emergency Department and Inpatient utilization costs for our health plan partners while improving quality of life and health outcomes for their members.

MedZed is seeking an accomplished Territory Manager to join our Operations Team and help advance our vision while contributing to long-term value creation.

Position Overview

The Territory Manager (TM) reports to the Regional Manager (RM) and supervises a team of up to 15 Community Health Navigators. The TM is responsible for leading their territory to achieve monthly and quarterly performance goals, including effective member outreach and engagement through door-to-door and telephonic outreach, timely completion of assessments and care plans, delivery of high-quality care, and successful member graduation.

The TM is also responsible for maintaining audit readiness by ensuring compliance with HIPAA, documentation standards, and all applicable state and health plan regulatory requirements.

A successful TM brings curiosity and a relentless drive for improvement, balances humanity with productivity, and demonstrates a genuine passion for leading and developing teams. The TM sets clear expectations, models effective behaviors, and maintains open and transparent communication.

The TM supports their field-based team through regular time in the field and by fostering strong relationships with community-based organizations, health plans, and other partners. The TM will teach, coach, and mentor CHNs to ensure escalation protocols are understood and consistently followed, member panels remain appropriately managed, safety expectations are met, and members receive exceptional care.

The TM may be on call during the scheduled work week and at other times as needed. Because the TM's schedule must remain flexible, the position requires the ability to adjust working hours to meet program objectives and operational needs.

Key Responsibilities

Team Leadership & Performance Management

• Lead and motivate a team of up to 15 Community Health Navigators to achieve and exceed monthly and quarterly performance goals.
• Manage day-to-day operations within the assigned territory.
• Conduct regular one-on-one meetings, performance reviews, and caseload reviews.
• Monitor individual and team productivity, member engagement, quality of care, panel management, and successful member graduation.
• Set clear performance expectations and hold team members accountable for meeting established goals and deadlines.
• Hire, onboard, manage, coach, and develop staff.
• Provide ongoing virtual and field-based coaching and professional development.
• Spend regular time in the field alongside CHNs to observe performance, reinforce best practices, and provide real-time coaching.

Operations & Member Engagement

• Ensure effective member outreach and engagement through field-based and telephonic strategies.
• Ensure timely completion of member assessments, care plans, interventions, and required documentation.
• Coach CHNs on developing Specific, Measurable, Achievable, Relevant, and Time-bound (SMART) goals and implementing appropriate interventions.
• Coach CHNs on overcoming member barriers, objections, and challenges to engagement.
• Ensure escalation protocols are understood, appropriately applied, and consistently followed.
• Problem-solve issues raised by CHNs in a proactive, creative, and solutions-oriented manner.
• Ensure team members maintain appropriate caseloads and effectively support members through successful program graduation.
• Ensure field safety expectations and operational protocols are consistently followed.

Clinical & Cross-Functional Collaboration

• Partner with Clinical Advisors to ensure coordinated, high-quality care delivery and alignment between operational execution and clinical priorities.
• Foster effective collaboration between Operations and Clinical teams to improve member outcomes, resolve barriers, and support continuous quality improvement.
• Promote a patient-centered and whole-person approach to member care.
• Support effective communication and escalation between field staff and clinical leadership.

Community & External Partnerships

• Develop and maintain effective relationships with community-based organizations, health plans, and other external partners.
• Identify and communicate community-based resources and opportunities in partnership with the Community Impact Partnership Program (CIPP) Director.
• Represent MedZed professionally within the community and with health plan and community partners.

Business & Financial Management

• Support program expansion, staffing requirements, and operational enhancements.
• Manage the Profit and Loss (P&L) for the assigned territory.
• Monitor territory performance against financial and operational targets.
• Ensure timely review and approval of payroll, expenses, and other compliance-related requirements within the territory.
• Identify opportunities to improve operational efficiency, productivity, and financial performance.

Compliance & Quality

• Maintain territory readiness for internal and external audits.
• Ensure compliance with HIPAA, documentation standards, state regulations, health plan requirements, and MedZed policies and procedures.
• Monitor documentation quality and timeliness across the team.
• Identify compliance risks and implement corrective actions as needed.
• Promote a culture of quality, accountability, safety, and continuous improvement.

Qualifications

• Minimum of 3–4 years of experience managing teams within health, social, community, or related services.
• High school diploma or equivalent required.
• Strong analytical skills with the ability to understand and effectively use data, dashboards, and reporting tools.
• Demonstrated ability to manage a Profit and Loss (P&L).
• Experience leading remote or geographically dispersed teams.
• Strong problem-solving and critical-thinking skills.
• Ability to adapt to constant change and work effectively under pressure in a fast-paced, challenging environment.
• Demonstrated ability to collaborate effectively across clinical, operational, and external partner teams.

Physical Requirements

• This position requires the employee to remain in a stationary position for prolonged periods while performing desk-based computer tasks.
• The employee must be able to constantly operate a computer, keyboard, and other office productivity machinery, such as a calculator, copy machine, and computer printer.
• The person in this position may need to occasionally move about inside the office/remote workspace to access file cabinets, office machinery, or other resources.
• The employee may occasionally be required to lift and move objects weighing up to 10 pounds (e.g., a laptop, files, or small office equipment).
• Clarity of vision at approximately 20 inches or less is required for working with computers and reading small print.
