• Conduct comprehensive psychosocial assessments through structured interviews, detailed observations, and thorough medical record reviews to evaluate emotional, social, and environmental factors influencing patient recovery.
• Deliver impactful counseling and emotional support to patients and their families, specifically targeting adjustment issues related to medical conditions, rehabilitation challenges, and transitioning into community living.
• Work in collaboration with a multidisciplinary rehabilitation team to design individualized treatment plans, with a strong focus on integrating psychosocial considerations into care strategies.
• Guide patients in navigating community resources, including support groups, vocational rehabilitation programs, and housing assistance, thus enabling them to access vital support systems.
• Educate patients and their families about available resources and services to ensure a seamless post-rehabilitation transition.
• Engage in discharge planning by coordinating with healthcare providers to facilitate transitions to home care or other settings as patients finish their rehabilitation journeys.

The population we serve is diverse, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Our clinical social workers play a vital role in helping these patients address the emotional and practical challenges associated with their health situations, ultimately promoting greater independence and quality of life.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.
• Strong communication and interpersonal skills, enabling effective interactions with patients and their families.
• A comprehensive understanding of adjustment issues, grief, and coping strategies pertinent to rehabilitation.

• Previous experience in a rehabilitation or medical setting, with a focus on delivering psychosocial services to patients.
