• Conduct thorough biopsychosocial assessments tailored to age and cultural backgrounds, identifying mental health, substance abuse, and medical needs within the primary care setting.
• Deliver high-quality behavioral health services, including counseling, intake evaluations, treatment planning, crisis intervention, and referrals to community resources for ongoing support.
• Document mental health evaluations, progress notes, and treatment plans accurately in electronic health records to ensure continuity of care and compliance with organizational policies.
• Facilitate individual and group therapy sessions, fostering a safe environment for clients to explore personal challenges and develop coping strategies.
• Work collaboratively with Quality Improvement teams to streamline data collection methods that enhance service delivery and patient outcomes.

• Must possess an active state license as a Licensed Clinical Social Worker (LCSW).
• A Master's degree in Social Work (MSW) is required, accompanied by a minimum of three (3) years of post-licensure experience in a clinical setting.
• Strong knowledge of counseling techniques, crisis intervention methodologies, and the ability to exercise independent judgment in clinical decision-making is essential.
• Exceptional communication skills and previous experience in group facilitation and therapeutic planning are highly valued.

• Experience working within integrated health care teams or community health settings is preferred, providing insight into multi-faceted patient care approaches.
• Familiarity with electronic health record systems and telehealth platforms will enhance your ability to perform efficiently in a remote setting.
