Working full-time, you will adhere to regular business hours throughout weekdays, with occasional evening and weekend shifts based on the needs of your patients and assigned caseload. • Conduct detailed psychosocial assessments by engaging with patients through interviews and observations, while also reviewing relevant medical records to identify emotional, social, and environmental factors affecting their recovery process.
• Offer counseling and therapeutic support to both patients and their families, effectively addressing emotional well-being and adjustment challenges common during rehabilitation.
• Collaboratively develop individualized treatment plans with the rehabilitation team, ensuring that all psychosocial needs are integrated into the overall strategy for patient care.
• Guide patients in accessing crucial community resources, including support groups, vocational rehabilitation services, and housing assistance, to facilitate a seamless transition back into daily life.
• Educate patients and families about available community resources and post-rehabilitation services, empowering them to take an active role in their recovery.
• Participate in discharge planning, ensuring patients have the necessary support and resources for successful transitions to home or alternative care settings.

You will work with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. This role requires empathetic engagement, as you help patients navigate the emotional and social complexities of their conditions while they work towards achieving their highest level of independence.

• Possession of a Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice.
• Strong interpersonal and communication skills to effectively interact with patients and collaborate with a multidisciplinary team.
• Comprehensive understanding of adjustment issues, grief, and effective coping mechanisms to guide patients through their recovery journey.

• Experience in a rehabilitation or medical setting is preferred, enhancing your ability to address the unique challenges faced by patients in recovery.
• Familiarity with community resources and support systems that can assist patients post-rehabilitation, allowing for effective referrals and follow-ups.

The salary for this position ranges from USD 75,000 to 100,000 per year, along with the opportunity for a sign-on bonus.
