• Conduct comprehensive clinical evaluations to assess patient needs and develop individualized care plans, in collaboration with the healthcare team.

• Lead process-oriented therapy sessions, providing a safe space for patients to express their feelings and experiences related to end-of-life issues.
• Coordinate with external referral sources and outpatient providers to ensure continuity of care throughout the patient’s journey.
• Manage crises effectively, employing de-escalation techniques and intervention strategies to maintain a safe therapeutic environment.
• Ensure discharge appointments are confirmed, prioritizing communication with patients and their families.
• Organize and participate in clinical outreach activities, representing Horizon Mental Care Services at community events and information sessions.

The primary focus of this role is on patients in hospice care—individuals facing serious and terminal health conditions, who require sensitive and comprehensive emotional support. You will work closely with patients and their families, addressing not only medical and psychological needs but also helping navigate grief, loss, and existential concerns associated with end-of-life transitions.

• A Master’s degree in Social Work from an accredited institution.
• Current and active LCSW license in the state of California.
• Strong written and verbal communication skills with the ability to express empathy and understanding in challenging circumstances.
• A well-demonstrated ability to establish and maintain healthy boundaries with clients.
• Experience in completing documentation and reporting in a timely and efficient manner, ensuring compliance with organizational standards.

• Previous experience working in hospice, palliative care, or a related setting, particularly with terminally ill patients.
• Familiarity with electronic health records and telehealth platforms to facilitate remote patient interactions.
• Demonstrated ability to foster therapeutic alliances with diverse populations, navigating cultural sensitivities and unique family dynamics.

We offer a competitive salary ranging from $120,000 to $130,000 annually, commensurate with experience and caseload.
