Training starting at $18.00 working in Brookdale communities and in-home care.
Interview and On-Board this week:
~PRN availability for 4, 8, and 12 hour shifts

Base pay in range will be determined by applicant's skills and experience. Role is also eligible for team based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company's 401(k) program.

Responsibilities
Provides direct care to external clients living outside the walls of a Brookdale Senior Housing Community, while promoting the client's highest level of functioning, as directed by the BAH Agency Leader and/or under the supervision of a licensed nurse when applicable by state rule. Assists in maintaining the client's environment in a safe and neat manner. Promotes quality services within non-skilled home care standards as well as applicable state home care agency regulations.
  1. Works independently in the client's personal home with supervision and support as per state regulations.
  2. Receives and communicates changes in the client's care needs and requested changes in specific home care services to the BAH Agency Leader as needed.
  3. Provides personal care and/or concierge/homemaker/companion services to the client per their individualized plan of care.
  4. Documents all pertinent information regarding care as assigned.
  5. Assists clients with activities of daily living, including bathing, dressing, grooming, toileting, transferring, and getting to and from activities and meals according to the client plan of care.
  6. Prepares residents for meals and assists to and from the dining areas. Assists with set up and feeding of meals.
  7. Assists residents with toilet and incontinence care. Makes incontinence checks at least every 2 hours and provides incontinent care as needed when providing care greater than 2 hours in a row.
  8. Keeps client's home neat and clean.
  9. Performs vital signs, B/Ps, weights and other trained procedures as per client's care plan.
  10. Completes prevention and restorative care that includes but not limited to PROM, AROM, ambulation, transferring, bathing, grooming, and other ADL care according to the plan of care.
  11. Maintains a working knowledge of the home care agency's policy & procedures pertinent to role.
  12. Assists in turning, positioning, and repositioning of clients.
  13. Communicates effectively and proactively with the clients, families, visitors and the BAH Agency Leader and scheduler.
  14. Actively participates in Community Quality Assurance Program.
  15. Maintains education development to include attending ongoing education development to maintain compliance with State and Federal Regulations and current knowledge of client care practices and maintains compliance with required annual training.
  16. Maintains client confidentiality.
  17. Assists clients with homemaker companion tasks such as laundry, shopping, light housekeeping, meals, according to the plan of care.
  18. Follows infection control and safety policies and guidelines.
  19. Maintains means to travel to client's homes to arrive on time for assignment.

Qualifications
Education and Experience
High school diploma or General Education Degree (GED) is required. Proof of completing an approved Certified Nursing Assistant program is required. Six months experience in working independently with clients in the general public in their own personal homes providing healthcare/homecare services is preferred.
Certifications, Licenses, and other Special Requirements
Successful completion of State C.N.A. course. Must maintain CNA certification. Reliable transportation is required. Must have a valid state identification card, with a valid driver's license being preferred.
