Home Health Nurse Responsibilities

  • Deliver skilled nursing care according to individualized care plans
  • Perform comprehensive assessments and ongoing patient evaluations
  • Administer medications, wound care, injections, and other clinical treatments
  • Educate patients and families on disease management, safety, and wellness
  • Collaborate with physicians, therapists, and the broader care team
  • Document all care accurately and timely in accordance with regulatory standards

Home Health Nurse Requirements

  • Preferred: Experience providing trach and respiratory care with precision and safety
  • Preferred: Experience with using a ventilator
  • Preferred: Experience with Infusions
  • Current unrestricted RN license in the State of Wisconsin
  • Preferred: Bachelor's degree in Nursing
  • Certified CPR and a negative TB skin test or chest x-ray
  • Minimum one (1) year of documented experience required in a home health agency, primary care clinic, or health facility, preferred
  • Possess exceptional nursing assessment skills
  • Analytic/Critical thinker
  • Strong clinical skills
  • Licensed driver with valid automobile insurance and reliable transportation
  • Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the client's mobility. Must be able to lift up to 50 pounds
