Job description

Obran Health is seeking an experienced RN Clinical Coordinator to join our team in a fully remote, temporary contract capacity. In this role, you will provide critical clinical coordination and operational support across both of our California-based, worker-owned home healthcare agencies: Physicians Choice Home Health and Apollo Home Healthcare.

Partnering closely with the Directors of Patient Care Services (DPCS), a team of Clinical Coordinators, and our field clinicians, your work will directly support our field clinicians, ensure our documentation is compliant, and help patients across both agencies receive high-quality care.

Key Responsibilities
  • Care Coordination & Oversight: Provide care coordination and clinical oversight for assigned patient caseloads across both agencies.
  • Clinical Documentation & Orders: Review, audit, and sign clinical documentation, including Starts of Care (SOC), Recertifications, Resumptions of Care (ROC), Discharges, and external physician orders.
  • Clinician Support & Triage: Along with the Clinical Coordinator team, serve as the point of contact for incoming clinical inquiries from field staff and patients, offering guidance and escalating complex issues to the DPCS as necessary.
  • Interdisciplinary Collaboration: Work directly with physicians, healthcare providers, and community partners to maintain continuity of care.
  • Quality & Compliance: Analyze clinical documentation and workflows to identify quality improvement opportunities and ensure alignment with agency policies and regulations.
  • Onboarding & Education: Support remote onboarding, training, and clinical guidance for new and existing field clinicians as needed

Requirements

  • Licensure: Active, unrestricted California Registered Nurse (RN) license
  • Home Health Experience: Minimum of 1-2 years of hands-on home health RN experience, with a deep understanding of home health clinical workflows
  • Supervisory Experience: Minimum of 1 year of supervisory experience in home healthcare or a similar field
  • Technical Skills: Strong proficiency with Kinnser/WellSky EMR strongly preferred
  • Remote Capability: Ability to fulfill the duties of this role remotely, with strong digital communication skills.

  • Pay Rate: $43-$48/hour, depending on experience and qualifications
