Remote Aging Care information

What is remote aging care?

Remote aging care refers to providing care, assistance, and support to elderly individuals through digital technologies, such as video calls, remote monitoring devices, and telehealth services. This approach enables caregivers and healthcare professionals to assist seniors in their daily activities, monitor their health, and respond to their needs without being physically present. Remote aging care can help older adults maintain their independence while ensuring their safety and well-being, especially for those who wish to age in place or have limited access to in-person care.

What are the key skills and qualifications needed to thrive as a remote aging care professional?

To excel in Remote Aging Care, you need a background in healthcare or social work, knowledge of geriatric care, and relevant certifications such as CNA, LPN, or equivalent experience in elderly support. Familiarity with telehealth platforms, remote monitoring tools, and care management software is essential for effective virtual support. Outstanding communication, patience, and empathy help build trust and rapport with seniors and their families. These skills ensure that aging clients receive personalized, safe, and effective care while living independently at home.

Remote Aging Care focuses on comprehensive health management and care planning for seniors, often requiring healthcare certifications. In contrast, Remote Elderly Support Specialists provide companionship and daily support, typically with basic caregiving skills. Both roles operate remotely within the senior care industry but serve different needs and require different qualifications.

What are the typical challenges faced by professionals in remote aging care and how can they be addressed?

Professionals in remote aging care often encounter challenges such as building rapport with clients virtually, ensuring clear communication with both clients and their families, and coordinating care across different locations and providers. To address these, it's important to use reliable technology platforms, establish regular check-ins, and maintain thorough documentation. Collaborating closely with other caregivers, social workers, and medical professionals helps create a comprehensive support system for aging clients, even when working remotely.

Healthcare Advocate

Baba

New York, NY โ€ข Remote

Part-time

Re-posted 11 days ago

Job description

Baba is seeking an experienced and compassionate Healthcare Advocate with Care Advocacy Experience to join our fast-growing care navigation and support team. If you're passionate about helping seniors, caregivers, and their families navigate the complexities of healthcare and aging, Baba is the place to make an impact with purpose.

About Baba

Navigating health and aging is hard. Baba makes it easier by pairing each patient with a real human advocate team that supports them between visits.

Baba already helps seniors and caregivers manage appointments, billing questions, and complex healthcare decisions. You’ll plug into that existing infrastructure—so you can focus on supporting patients, not chasing no-shows or doing endless admin.

Baba is backed by top tier investors and supports thousands of families across assisted living, nursing homes, and at-home settings. www.callbaba.com

About the Role

As a Healthcare Advocate at Baba, you will play a vital part in enhancing the healthcare experience for our clients. Using your care advocacy skills and experience, you’ll offer guidance and support to patients and their families—helping them access care, clarify provider instructions, connect with community resources, and reduce barriers to a healthier, more independent life.

This is a remote, 1099 (contractor) role. You may choose part-time or full-time engagement.

NOTE: Minimum 20 hours of availability per week (Monday-Sunday), including at least 75% during standard business hours (Monday–Friday, 9:00 AM–5:00 PM).

What You’ll Do
  • Develop trusting relationships with patients, grounded in listening, respect, and clarity.

  • Assess the whole person — medical, social, and environmental factors — to pinpoint barriers and needs.

  • Create individualized care plans that integrate medical, social, and behavioral supports.

  • Support patients across a range of needs, including:

  • Connecting to local services (transportation, meal delivery, home care, social supports)

  • Analyzing medical bills and helping with claim appeals or explanations

  • Identifying medication cost savings, co-pay assistance, or formulary options

  • Linking to social support networks (caregivers, community programs, peer groups)

  • Organizing and maintaining medical records, care histories, and provider communication

  • Coaching and supporting patients on best practices for managing conditions and navigating care

  • Document interactions, observations, and patient progress in Baba’s systems with clarity and timeliness.

  • Collaborate with clinical, product, and operations teams to optimize workflows, escalate complex issues, and refine the patient experience.

  • Provide feedback on platform features, tools, and processes to improve Baba’s care copilot capabilities.

What You Bring
  • Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or a related healthcare field.

  • Active RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management credential preferred.

  • 2+ years of experience in care navigation, patient advocacy, care management, case management, home health, discharge planning, utilization management, or clinical social work, preferably supporting older adults or Medicare populations.

  • Solid knowledge of Medicare rules, eligibility, benefit structures, claims, and appeals.

  • Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse and underserved populations.

  • Empathy, patience, and strong communication skills— you can explain complex things simply.

  • Comfort working with digital tools, EMRs / care-management software, and adapting to new tech.

  • Commitment to continuous improvement— you’ll raise issues, suggest solutions, and help refine Baba’s model.

  • Organized, proactive, and able to juggle multiple priorities without losing quality.
