Gotham Enterprises Ltd is seeking a Licensed Clinical Social Worker (LCSW) for a remote, full-time position to provide critical support and care to seniors and their families at our Hospice Center in Peachtree City, Georgia. This position is structured for remote work, allowing flexibility while ensuring that our clients receive uninterrupted support. Although primarily remote, occasional visits to clients’ homes or care facilities may be necessary. Your role will involve coordinating with interdisciplinary teams, including nurses and caregivers, to address the needs of clients effectively. A full-time schedule typically runs from Monday to Friday, between the hours of 8:00 AM and 4:30 PM, with potential evening or weekend hours based on client requirements.

• Conduct in-depth assessments of seniors' psychosocial needs, taking into account emotional, mental, and social factors that may impact their overall well-being.
• Develop personalized care plans in partnership with clients and their families, ensuring alignment with their personal goals and preferences.
• Offer counseling and emotional support tailored to help clients navigate significant life transitions, such as health crises or changes in living arrangements.
• Advocate for client rights, ensuring they have access to community resources and necessary support services.
• Collaborate with a diverse range of healthcare professionals to ensure coordinated and comprehensive care delivery.
• Perform regular home visits to assess living conditions, monitor client well-being, and address safety concerns promptly.
• Educate clients and their families about available resources, support groups, and programs to foster social engagement and enhance quality of life.
• Maintain thorough and accurate documentation of assessments, care plans, and progress notes using electronic health record (EHR) systems.
• Stay updated on best practices, policies, and regulations in social work to ensure high ethical standards and compliance in your practice.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Georgia.
• A minimum of two years of experience in social work, ideally within a community or geriatric setting.
• Strong understanding of social work principles, aging-related concerns, and available community resources.
• Excellent skills in assessment, counseling, and problem-solving related to senior care.
• Demonstrated ability to empathize with clients and build trusting relationships with seniors and their families.
• Effective communication and interpersonal skills to collaborate successfully with interdisciplinary teams.
• Familiarity with electronic health records (EHR) systems and a working knowledge of relevant software applications.

• Prior experience working in a hospice or palliative care environment.
• Knowledge of local community resources and support services available to seniors.
• Experience in crisis intervention and conflict resolution among family caregivers.
